Slow Mohs Surgery
Dr. Pennie’s expertise in Mohs surgery, particularly her experience performing it in multiple sessions, distinguishes her from other dermatologists. This more drawn out skin cancer excision is affectionately referred to as Slow Mohs. Unlike regular Mohs surgery, Slow Mohs involves a multi-stage approach, utilizing special staining techniques to provide detailed examination of each tissue sample. The samples are processed overnight and stained in the morning, allowing for enhanced precision in identifying cancer cells. The Slow Mohs technique is used for cases of melanoma and melanoma in situ, where cancer cells are more difficult to see. The procedure is especially suited for delicate areas such as the face, hands, and feet, where the skin is less lax and more difficult to repair.
Typically initiated 2-4 weeks following diagnosis the process involves excising and staining each stage of tissue. If melanoma cells are detected, another stage is taken, and this continues until clear margins are achieved—most patients only require one or two sessions. This method has the highest cure rate due to its specificity in assessing margins.
Dr. Pennie stands out in her field because while many dermatologists may excise the cancer, they often do not perform the skin reconstruction and instead refer out to a plastic surgeon. Throughout the recovery process, patients are advised to keep the bandage dry for the first 48 hours, with some swelling and bruising expected. Patients are instructed to limit activities for a week, after which stitches can be removed. While movement is encouraged to promote blood flow, patients should avoid strenuous activities to aid in healing. Following stitch removal, scars may appear swollen or red, and the use of scar gel and sun protection is recommended. It typically takes a couple of months for the scar to achieve its best appearance. At this time, Dr. Pennie may recommend a laser treatment for the scar to optimize results.
